Replacement Certificates

Lost or replacement certificates can be mailed to you or scanned and emailed but due to the large amount of request we are now charging a $10.00 charge

Please fill out the form for replacement certificate and then pay $10.00

We can either mail it to your home or scan and email the certificate for you.

Please furnish us with the following:

Full name and address
What is the date of the lecture
What is location of conference (name of hospital)
Your AARC # is a member
